Is There an Urge to Operate the Patients with Severe Carpal Tunnel Syndrome? A Year of Neurophysiological Follow Up of Severe CTS

چکیده مقاله:
Abstract OBJECTIVES: Several approaches have been proposed for the treatment of carpal tunnel syndrome (CTS) based on its severity. The aim of this study is to determine the optimal management for severe CTS. METHODS: This study was conducted in a cross-sectional fashion during a 22-month period from August 2017 to June 2019 in a referral rehabilitation clinic in southern Iran. A total of 117 hands diagnosed with severe CTS were included in 78 patients according to Electro-diagnostic study (EDX) results (considering Sensory Proximal Latency (SPL)>3.6 msec, Sensory Distal Latency (SDL)>5.3 msec, Sensory Nerve Conduction Velocity (SNCV) <30 m/sec, Distal Motor Latency (DML)>6.5 msec as being severe) who refused to undergo surgery. Boston questionnaire was filled out and conventional EDX was carried out at the first and at the 1-year follow-up visits. RESULTS: In patients with detectable sensory nerve action potential (SNAP), motor amplitude (P<0.002) and latency (p<0.01), SPL (p<0.003) and SNCV (p<0.006), and Boston parameters improved significantly in the 1-year follow-up visit compared to results at first visit. However, improvement in patients with absent or low amplitude SNAP at first visit was only observed in proximal sensory latency (p<0.005) and amplitude (p<0.003). CONCLUSIONS: There is a considerable chance for non-surgical improvement of patients with severe CTS in terms of symptom relief, hand function and EDX parameters in those with detectable SNAP at first visit; however, patients with undetectable SNAP have little, if any, chance for improvement with conservative measures.

Pick-up test: an indication for opponensplasty in patients with severe carpal tunnel syndrome.
PURPOSE To use a pick-up test to assess thenar motor deficit and results of opponensplasty. METHODS Eight consecutive patients with idiopathic severe carpal tunnel syndrome and severe thenar motor deficit were identified. All were females aged 39 to 60 (mean, 53) years and right-hand dominant. The severity of thenar motor deficit was assessed using the pick-up test. Failure to pick up a coin ...
متن کاملHenna stained palms in patients with carpal tunnel syndrome (CTS)
Carpal tunnel syndrome (CTS) is the most common diagnosed upper limb compressive neuropathy; though its diagnosis remains controversial. Some patients apply Henna (the extracts of the plant Lawsonia inermis) to their palms to relieve the symptoms. We don’t know how common is this practice and if it does help in clinical diagnosis. The purpose of this study was to examine the diagnostic value of...

Background: Carpal tunnel syndrome (CTS) is characterized by complications such as pain, paresthesia, andnumbness in the fingers. There are some surgical therapies for the management of severe carpal tunnel, but differencesexist between the treatments available for creating the opposition. متن کاملPostures of upper extremity correlated with carpal tunnel syndrome (CTS).
OBJECTIVES Non-medical hospital staff members are in frequent contact with patients and therefore are required to perform a wide variety of repetitive and high-frequency activities. The objective of this study was to assess the relationships between upper extremity activity and carpal tunnel syndrome (CTS) among non-medical hospital staff members. MATERIAL AND METHODS Carpal tunnel syndrome i...
متن کاملCarpal tunnel syndrome despite negative neurophysiological studies.
The purpose of the present study was to compare the results of conservative and operative treatment for patients with carpal tunnel syndrome having normal neurophysiological studies. We studied 125 patients with normal neurophysiological studies and analysed eight symptoms and signs as "prognostic factors". Ninety-six patients were treated conservatively (splintage, steroid injection, antiinfla...
